Job Description

STATEMENT OF PURPOSE

The pharmacist ensures safe, effective, and efficient medication use within the hospital.

MAJOR DUTIES AND RESPONSIBILITIES

Review each patient’s medication chart or prescription for the appropriateness of the drugs prescribed and to contact the prescribing doctor when an intervention is needed. The review must ensure that:

  • All orders are clear and specific
  • Doses are prescribed within therapeutic limits
  • Any potential adverse reactions and interactions are identified
  1. Fulfill all drug orders accurately and in a timely manner.
  2. Monitor drug levels (where applicable) and recommend dose adjustments accordingly.
  3. Ensure that all related costs and revenue are accounted for, and accurately recovered.
  4. Dispense medications to patients and explain the indications for drug therapy and instructions for administration.
  5. Provide drug information to other healthcare professionals.
  6. Check and release lab compounded products, and repackaged unit doses.
  7. Delegate, oversee and review the work of assigned subordinates.
  8. Ensure that all sales and supply of medicinal products comply with legal requirements:
  • Maintain all records pertaining to drug receiving, supply, storage, and distribution as required by the hospital and/or legal authorities.
  • Perform inspections of drug storage areas in the hospital.

DRUG INFORMATION

  1. Provide drug information service to other healthcare professionals and patients
  2. Assist in preparing and updating drug information tables/charts.
  3. Assist in updating drug lists for various types of funding/subsidies.
  4. Develop patient education materials.
  5. Assist in the preparation for Pharmacy and Therapeutics (P&T) Committee meetings, including writing of drug reports, following up on pending issues with applicants, and post-meeting work, including new drug update posters for newly approved drugs
  6. Assist in evaluation and follow-up actions for Named Patient Basis (NPB) drug requests
  7. Perform product comparison and evaluation of drug alternatives or drugs of different brands/packaging for drugs with supply disruption or discontinuation
  8. Participate in medication use evaluation projects
  9. Assist in the reviewing of the setup of drugs (new drugs/changes) in pharmacy dispensing system/billing system.

b) PHARMACY INFORMATICS

1. Work closely with Pharmacy Informatics team lead, pharmacy staff and section in-charges to continuously fine-tune workflow and error-proof processes.

2. Implement and assist to manage pharmacy-, hospital-related IT systems and automation by:

  • Gathering requirements with key stakeholders in the front-line.
  • Assisting in overseeing the work of Pharmacy Technican Executive in the Pharmacy Informatics team.
  • Participating in user acceptance testing of systems
  • Conducting IT-related training sessions for pharmacy staff

3. Maintain and provide support for pharmacy and hospital-related IT systems and automations by:

  • Ensuring that systems are working well after implementation
  • Investigating and monitoring issues reported, as well as keep track of outstanding deliverables

4. Provide assistance to pharmacy staff with IT-related issues such as database handling to help in pharmacy administration, miscellaneous IT queries, pharmacy billing matters and ad-hoc IT requests.

5. Perform system impact analysis while managing any system implementation or change request. The analysis includes ensuring patient safety and medication charges (patient charging and all related costs and revenue are accounted for, and accurately recovered).

GENERAL

1. Contribute to pharmacy education and professional development:

  • Deliver lectures on relevant topics to pharmacy staff and other hospital personnel
  • Engage in continuous learning through journals and publications to stay current in the field

2. Ensure compliance with departmental, hospital, and external regulatory policies and practices related to pharmacy operations.

3. Mentor and train pre-registration pharmacists and pharmacy students.

4. Actively support hospital initiatives, not limited to:

• Quality Convention

• Community Outreach Programmes

• Staff Welfare Activities

• Recreational Events

5. Participate in clinical research and quality improvement initiatives.

Requirements;

  • Bachelorof Science degree in Pharmacy from an accredited University recognized by the Singapore Pharmacy Council
  • Registeredwith the Singapore Pharmacy Council
  • Valid practicing certificate
