1. Information & Referral
i. Attend to walk-ins and telephone enquiries. Refer to the relevant staff for follow up if necessary.
ii. Conduct intake assessments.
iii. Provide appropriate information and refer clients to relevant agencies for further intervention.
iv. Manage intake administration.
2. Casework and Counselling
i. Manage assigned cases and case referrals.
ii. Conduct assessment, planning and intervention for cases.
iii. Develop and review appropriate intervention strategies.
iv. Participate in and/or conduct case conferences.
v. Evaluate and document case outcomes.
vi. Complete case recordings and update client information and documentation in the case management system.
3. Group Work & Community Work
a. Programme Planning & Implementation
i. Support the preparation, planning, development and implementation of relevant programmes to respond to the needs of clients and maintain proper documentation.
ii. Support the coordination and pooling of appropriate resources in the community.
iii. Network and collaborate with other community stakeholders.
b. Programme Evaluation
i. Evaluate and document clients’ progress and programme outcomes.
ii. Identify trends, gaps or changes needed in provision of Centre’s services and programmes in the community.
4. Administrative Functions
i. Support social workers/counsellors in casework matters when necessary.
ii. Work together with the admin team members to support and facilitate the Centre’s administrative matters.
iii. Generate, compile and analyse required data from SSNet One and other portals for casework for the Centre’s operations and reporting/ submission.
iv. Update records on community resources.
v. Provide guidance and support to student interns and volunteers where appropriate.
5. Undertake any other responsibilities and duties assigned.
